Tips for Downloading, Signing, and Saving Fillable PDFs
Save the form to your computer before completing it.
You must download and save the PDF form to your computer or network drive first and then open it with Adobe Reader or Adobe Acrobat in order for the sign and submit buttons to work properly. Please save prior to filling the form and then again AFTER you hit the submit button. When you hit the submit button an email will pop up with the appropriate email address. At this point any attachments such as receipts can be added and sent along with the form.
